    • Profile

Home and Community offers a variety of at-home and community services to people with acute, chronic, palliative or rehabilitative health care needs. Read our Guide to Home and Community Care to learn more. We support a philosophy that home, with appropriate supports, is the best place to recover from illness and injury, manage chronic conditions and live out final days. We offer both short and long-term services to help you remain independent and in your home.

If you or a family member want to receive Home & Community services, you must meet the eligibility requirements.

Call the Home & Community Care office in your area for more information.

Note: This service is available across Interior Health Authority. The address provided here is that of the health authority’s corporate office and has been used for general mapping purposes only. It does not represent the actual physical location of the service.
