The IRDP provides an intensive 4-6 week, team oriented program that offers an alternative to inpatient care for individuals with newly diagnosed neurological or musculoskeletal impairments who require intensive interdisciplinary rehabilitation.
Referrals must be made by a physician or nurse practitioner.
Complete the GFS Outpatient Referral Form and the Rehabilitation Checklist. Links to these forms can be found on the website.
