  • Profile

Community Rehab Early Discharge Initiative (REDi) is delivered in outpatient clinics (attached to hospitals) in six Fraser Health communities. It is an outpatient service providing coordinated rehab to clients who have realistic, achievable functional goals, who no longer need to stay in hospital and can be supported at home. It includes three components:

    1. Central intake process that coordinates services and defines a clients’ unique care plan
    2. Outpatient clinics delivering interdisciplinary rehabilitation
    3. Community reintegration programs such as TIME (Together in Movement and Exercise) classes and Stroke Education classes

A referral is required in order to access the program. Check with your health professional to see if you may be eligible. One of the most important criteria a client must meet is to have identifiable functional goals for rehabilitation that are highly sensitive to improvement with outpatient rehabilitation.

    Located in: Fraser Health Authority.

Steve, a stroke survivor, shares his experience recovering from a series of strokes using speech and language therapy in Fraser Health’s REDi Program.
