Fraser Health, Outpatient Rehabilitation Clinics - Chilliwack General Hospital

Outpatient Rehabilitation Clinics focus on enabling people to self-manage their rehabilitation goals, improve their ability to participate in daily activities and improve their quality of life after discharge from the hospital or inpatient rehabilitation unit.

A referral is required in order to access the program. Check with your hospital or community health provider to see if you may be eligible. To access this service, you must meet all of the criteria.
